The rate and the amount live on the expense, not on a separate sheet. Working out the quarter's input VAT stops being an afternoon with a calculator.
Reimbursable expenses
Flag what you're owed, record the repayment when it lands, and keep what's still outstanding in plain sight.
Multiple currencies
Record what you paid in the currency you paid it in, and see the equivalent. Totals never add pounds to pesos, because that isn't a total.
Transfers between accounts
Moving money from one account to another isn't spending it. It's recorded for what it is and stays out of the monthly figures.
